Best Practices for Coordination and Transitions of Care Summit – September 15, 2021

This summit will address best practices during the transition from pediatric to adult care; provide prospective and guidance on the challenges patients and families face during the transition; identify strategies to help patients maintain continuity of care and adhering to treatment; and discuss financial and benefit considerations prior to transitioning to adult care.
